The process of making a home accessible often can be overwhelming to a homeowner, but Elements of Home is here to simplify it into manageable, stress-free steps and turn it into reality!

We collaborate directly with private-pay homeowners and also assist clients working through a caseworker, seeking county-waivered services (such as those on DD, CADI, CAC, EW, AC, and BI waivers). Many of our clients have a case manager; if you are interested in working with Elements of Home, please talk to your case manager about the potential for home modifications and then have your case manager complete a Referral Form.


STEP 1

Elements of Home LLC provides a comprehensive range of Minnesota home modification services, all of which begin with an onsite Environmental Home Access and Safety Assessment. This happens within one week of receiving the referral form from your case manager. While each case is unique, the initial step involves one of our dedicated team members visiting your home for a consultation to discuss your needs.

Following our visit, our recommendations will be sent to your case manager within 48 hours of the home assessment visit.

 


STEP 2

Once County approval is obtained, we’ll draft a personalized scope of work containing detailed information based on our recommendations and your specific needs.

After the scope of work receives County approval, we arrange a pre-construction meeting with the selected contractor, Elements of Home project coordinator, and the client to ensure that all details of the project are thoroughly understood and agreed upon.


STEP 3

As your project coordinator and advocate, we guide you through the entire process. Ensuring a successful home modification project is our utmost priority, and we achieve this through clear, consistent, and supportive communication.
